Important Notice Regarding Form 1095-C
In accordance with IRS Bulletin 2025-11, the City of Malibu will no longer automatically furnish Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) to individuals.
Employees who wish to receive a copy of their Form 1095-C may request one at no cost. Upon request, the form will be provided within the timeframe required by the IRS.
Form 1095-C is not required to be filed with individual tax returns; however, employees may retain the form for their personal records.
